Program Policy
Registration:
Registration for all programs will be taken on a first come, first served basis. Valid membership cards must be presented at the time of registration. Payment must be made at the time of registration. An incentive for early registration is implemented (excluding programs with an *) with a 10% discount valid on registration nights only.
Payment:
Payment may be made in cash, cheque, debit, or credit card. Cheques to be made payable to CRESCENT TOWN CLUB INC., post dated cheques are only accepted on approval of management. A separate registration form and cheque is required for each participant.
Refund & Cancellation:
All requests for refunds must be received in writing prior to the second class of the program. Crescent Town Club Inc. will only issue refunds for programs that are cancelled, and for medical reasons when a doctor’s note is provided.
Credit:
Credits will be issued for all other “refund” situations. They will be prorated, and may be subject to a 20% service charge. All credits must be approved and are valid for 12 months from the date of issue. Credits may only be applied against future program registration.
PAR-Q questionnaire:
The Par-Q Questionnaire (available at the Club) may be completed by program participants aged 15 to 69. Program participants must answer all the questions. If a participant answers YES to any of the questions they must provide a doctor’s note giving them clearance to take part in the intended programs. Participants must inform the Club Management if their health changes. (i.e. that you would answer YES to any of the questions.) If you are over 69 years of age and /or not used to being very active, check with your doctor.
Medical Services:
Crescent Town Club Inc. will, at its sole discretion, seek medical attention for any program participant as required and will endeavor to contact that participant’s emergency contact. Crescent Town Club Inc. will not assume responsibility for any medical costs, including ambulance services that may arise from participation in any program.
Private/ Semi- Private Lessons by Appointment:
Cancellations must be made in writing and submitted to the Crescent Town Club reception no later then 24 hours prior to the scheduled appointment. Written cancellation must contain the participants’ name, telephone number, type of lesson, instructor’s name, and the scheduled date and time of the appointment to be cancelled. Make up appointments are to be rescheduled at the convenience of the instructor and the participant.
If the participant fails to attend the scheduled appointment without written notice as stated above, the appointment will be considered a “no show”, will not be rescheduled, and the participant will be charged for that appointment s if it had occurred.
Instructors will wait a period of 15 minutes for late participants. After the waiting period the appointment will be treated as a “no show”.
